An A+ recorded above 4.0 is usually accompanied by a cap on the cumulative average at 4.00.
At several institutions here the A+ and the A are both 4.00, so the plus is a transcript note rather than extra grade points.
This is a property of the grade scale, and is not the same mechanism as a weighted high-school GPA.
